As a Registered Nurse with at least 2 years of home-based healthcare experience within the last 10 years, you will be responsible for conducting comprehensive patient assessments, developing and implementing individualized care plans, and providing education and support to patients and their families.
Responsibilities:- Conduct in-home assessments of patients in a community-based setting
- Develop and implement individualized care plans that meet the unique needs of each patient
- Provide education and support to patients and their families to promote healthy living and disease management
- Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes
- Maintain accurate and detailed records of patient assessments, care plans, and progress
