Premier Home Health Providers Inc. seeks a caring LPN/LVN to provide direct home-based patient care under the supervision of a Registered Nurse. You will implement individualized care plans and educate families, traveling to patient homes to deliver safe, quality nursing services.
Applicants must hold an active California LPN/LVN license and CPR certification with AHA, have 3+ years of nursing experience (home health preferred), and possess a valid driver’s license with reliable transportation.
