Elevance Health is seeking a Telephonic RN Nurse Case Manager I (Ameri Ben) to assess, develop and coordinate care plans for members with complex needs. The role can be done virtually with required in-person training, and involves telephonic case management and possible on-site discharge planning.
Minimum BA/BS in health-related field with 3 years clinical experience and an unrestricted RN license are required; multi-state licensure is needed for cross-state work.
