The Nurse Practitioner provides home-based primary and palliative care to elderly, frail, chronically ill, and homebound patients in independent living, assisted living, and residential home settings within an assigned geographic territory.

Established patient base and interdisciplinary care team

What You’ll DoPerform comprehensive medical evaluations and provide ongoing primary and palliative care.

Manage chronic conditions, symptoms, medications, pain, and quality-of-life concerns.

Develop and regularly review individualized care plans with patients, families, and caregivers.

Order and interpret diagnostic testing as clinically appropriate.

Prescribe medications and treatments within scope of practice.

Educate patients and families regarding treatment options, advance care planning, hospice, and supportive services.

Collaborate with nurses, social workers, hospice teams, caregivers, specialists, and other members of the care team.

Coordinate safe transitions between home, hospital, skilled nursing, hospice, and other care settings.

Maintain accurate and timely EMR documentation, including completion within 72 hours of visits.

Provide coverage for other Practitioners within the care group as needed.

Maintain compliance with organizational policies, professional standards, and regulatory requirements.

Participate in continuing education and professional development.
