St. David's Medical Center seeks a Home Health Assistant Director to support leadership of our home health services. Reporting to the Director, this RN leader oversees daily operations, care coordination, and clinical quality for a growing home-based patient population. Responsibilities include supervising field staff, monitoring compliance with regulatory and agency standards, managing performance and scheduling, and implementing evidence-based practices. The role partners closely with physicians, case managers, and families to ensure safe transitions of care, exceptional patient outcomes, and a culture of compassionate, patient-centered service.
Responsibilities
- Oversee daily operations of home health services to ensure safe, high-quality patient care
- Supervise, mentor, and evaluate home health nurses and clinical staff
- Coordinate patient admissions, care planning, and transitions from hospital to home
- Monitor compliance with federal, state, and accreditation standards for home health
- Analyze quality, utilization, and patient satisfaction data to drive performance improvement
- Collaborate with physicians, case managers, and families on individualized plans of care
- Assist with budgeting, staffing models, and resource allocation for the service line
- Develop and implement clinical protocols, policies, and staff education initiatives
- Support survey readiness and respond to audits, incident reviews, and corrective actions
- Promote a culture of compassionate, patient-centered care and teamwork
