Home Health Assistant Director

St. David's Medical Center seeks a Home Health Assistant Director to help lead our expanding home health services. Reporting to the Director, you will oversee clinical operations, staff scheduling, quality initiatives, and regulatory compliance for a diverse home-based patient population. Responsibilities include supervising RNs and support staff, coordinating interdisciplinary care, managing performance metrics, and supporting budget and policy development. You'll champion patient-centered, compassionate care while fostering a high-performing, collaborative team culture and driving best practices across our home health program.

Responsibilities

• Oversee daily clinical operations of the home health program
• Supervise, mentor, and evaluate nursing and support staff
• Ensure compliance with state, federal, and accreditation regulations
• Coordinate interdisciplinary care plans for home-based patients
• Monitor quality, safety, and patient satisfaction metrics
• Assist with budgeting, staffing models, and resource allocation
• Develop and update policies, procedures, and best practices
• Lead staff education, onboarding, and performance improvement initiatives
• Collaborate with physicians, case managers, and community partners
• Promote a culture of compassionate, patient-centered care
