Position: Pediatric Home Health Registered Nurse (RN)

Job Summary

This is a remote, field-based position
. Performance Home Health is seeking a compassionate and experienced Pediatric Home Health Registered Nurse (RN) to provide skilled, family-centered nursing care to pediatric patients in their homes. The RN is responsible for comprehensive pediatric assessments, skilled nursing interventions, medication management, patient and caregiver education, care coordination, and timely clinical documentation. This role collaborates with physicians, case management, clinical leadership, and other members of the interdisciplinary care team to promote safe, consistent, high-quality patient outcomes.

Responsibilities
  • Perform comprehensive pediatric nursing assessments and monitor changes in patients’ clinical conditions.
  • Provide skilled nursing care in accordance with physician/provider orders and the established plan of care.
  • Administer medications and treatments safely, including appropriate weight-based medication verification.
  • Provide wound care, vascular access/device care, infusion therapy, and laboratory specimen collection as ordered and within RN scope of practice.
  • Monitor growth, development, nutrition, pain, vital signs, and overall patient safety.
  • Identify and promptly escalation significant changes in patient condition to the appropriate provider and clinical team.
  • Educate parents and caregivers regarding medications, treatments, disease management, infection prevention, safety, and warning signs requiring medical attention.
  • Assess the home environment and caregiver readiness to support safe pediatric care.
  • Coordinate care with physicians, case managers, therapists, pharmacies, laboratories, pediatric specialists, and other healthcare professionals.
  • Complete accurate and timely clinical documentation in the agency’s electronic medical record.
  • Follow infection-control, patient-safety, HIPAA, and agency compliance requirements.
  • Maintain professional communication with patients, families, caregivers, and internal clinical teams.
  • Participate in assigned patient coverage and meet applicable productivity expectations. Adult patients may also be required to be seen in order to meet productivity expectations.
Required Experience
  • Current, unrestricted Registered Nurse (RN) license in Texas or valid Nurse Licensure Compact multistate privilege.
  • Must have at least 1 year of home health experience
    .
  • Minimum of one year of pediatric nursing experience required.
  • Current CPR/BLS certification.
  • Strong clinical assessment, critical-thinking, and communication skills.
  • Ability to provide skilled nursing care independently in a home-based environment.
  • Valid driver’s license, reliable transportation, and ability to travel throughout the assigned service area.
Preferred Experience
  • Experience caring for pediatric patients with complex medical needs preferred.
  • Experience with wound care, infusion therapy, vascular access devices, and patient/caregiver education is a plus.

This is a Remote Field-Based position providing direct patient care in patients’ homes throughout the assigned coverage area. The RN must be comfortable traveling between patient locations and working independently while maintaining close communication with the interdisciplinary care team.
