** About the Role
** We are seeking a licensed and experienced Practical Nurse (LPN) to join our healthcare team. Under the direction of a Registered Nurse (RN), the LPN will provide efficient and effective patient care while ensuring positive clinical outcomes and patient satisfaction. This role operates within the scope of practice defined by the State Nurse Practice Act and involves direct patient care, care planning, and supervision of unlicensed staff.
** Key Responsibilities**   – Patient Evaluation:
Collect, report, and document patient conditions and report changes to the RN   – Care Planning:
Assist in developing and implementing individualized patient care plans to improve health outcomes   – Direct Patient Care:
Administer medications and treatments as per physician orders   – Care Management:
Coordinate discharge planning, communicate recommendations, and educate patients and families   – Supervision of Staff:
Monitor and assign duties to unlicensed healthcare staff as per regulations

** Qualifications & Requirements**   – Graduate of an approved Practical/Vocational Nursing program   – Valid LPN or RN license in the state(s) of practice   – Experience in rehabilitative or geriatric nursing preferred   – Must provide verification of TST (tuberculin skin test) as required by law   – Current BLS/CPR certification (per state and facility requirements)   – Valid driver’s license or reliable transportation   – Must be able to lift up to 50 lbs and stand or walk for long periods   – Compliance with HIPAA, Federal False Claims Act, and healthcare regulations
