Amcare Pro Home Health, Palliative Care & Hospice is seeking an experienced and highly organized LVN Intake and Scheduling Supervisor to oversee intake, referral, staffing, and scheduling operations across our home health, palliative care, and hospice services.

This role ensures referrals are reviewed promptly, patients are assigned to qualified clinicians, required documentation is complete, and services begin within established time frames. The supervisor will lead and support the intake and scheduling team while collaborating with clinical leadership, field clinicians, referral sources, authorization staff, patients, caregivers, and other departments.

Key Responsibilities
  • Collaborate closely with the Operations Manager, Director of Nursing, and Clinical Managers.
  • Review incoming referrals and complete clinical reviews.
  • Verify required documentation, including valid physician orders, face-to-face documentation, and history and physical reports.
  • Review supporting documents, including Medicare Secondary Payer information and workers’ compensation claims.
  • Oversee infusion patients and coordinate appropriate clinical services.
  • Monitor hospitalizations and ensure timely SOCs, ROCs, recertifications, evaluations, and discharges for all disciplines, including contract therapy.
  • Assist with care coordination, patient advocacy, care navigation, visit-frequency oversight, and resolution of patient concerns.
  • Support marketers with clinical and service-related questions, including PCP referrals.
  • Coordinate with hospital case managers and referral sources as needed.
  • Maintain an accurate patient census and discharge reports for both branches.
  • Assist with documentation required for PCR submissions and Notice of Admission processing.
  • Support the Operations Manager with obtaining physician signatures on orders.
  • Assist schedulers with staffing challenges and provide ongoing education and guidance.
  • Review patient charts for scheduling errors and coordinate timely corrections.
  • Run weekly episode continuity reports to identify inaccurate episodes, missing visits, or plotting errors.
  • Coordinate discharge and readmission processes related to insurance changes.
  • Ensure SOCs, evaluations, ROCs, recertifications, and discharges are completed correctly and on time.
  • Coordinate services with home-visiting physicians, wound care clinics, and other healthcare providers.
  • Perform other related duties as assigned.
Qualifications
  • Current and unrestricted Texas LVN license.
  • Minimum of two years of LVN experience.
  • Home health, palliative care, hospice, intake, scheduling, or care coordination experience preferred.
  • Previous leadership or supervisory experience preferred.
  • Knowledge of admission requirements, physician orders, plans of care, visit frequencies, and insurance authorization processes.
  • Experience with Well Sky/Kinnser or a similar electronic medical record system preferred.
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Ability to manage multiple priorities and meet time-sensitive deadlines.
  • Proficiency with electronic medical records, scheduling systems, email, and Microsoft Office.
Work Arrangement

This is a full-time hybrid/remote position. The selected candidate must maintain a secure and private workspace, reliable internet and phone access, and consistent availability during assigned working hours. Occasional attendance at the Richardson office, meetings, training sessions, and other in-person activities will be required.
