Nurse Practitioner - Primary Care

Role: Nurse Practitioner providing home-based primary care for medically complex, home-limited adults.

• Perform in-home and telehealth visits to assess patient needs.
• Manage chronic and acute conditions through comprehensive evaluations.
• Coordinate care with an interdisciplinary team to ensure optimal patient outcomes.
• Order and interpret diagnostic tests and prescribe medications accordingly.
• Educate patients and caregivers to promote health and prevent hospitalizations.
• Document all clinical interactions in the EHR and support quality improvement initiatives.
• Contribute to expanding access to compassionate primary care for vulnerable populations.

• Current Nurse Practitioner licensure and relevant clinical experience.
• Strong assessment and clinical decision-making skills.
• Ability to manage multiple conditions in a home-based setting.
• Excellent communication and patient education abilities.
• Comfort with using telehealth platforms and electronic health records.
• Dedicated to providing patient-centered, compassionate care within a mission-driven environment.
