Elara Caring is seeking a Clinical Team Specialist – RN (Per Diem/Remote) to join our home-based care team. This role focuses on reviewing and approving care plans, conducting OASIS reviews, ensuring timely visits, and upholding compliance with state and federal requirements from a remote home office.
The ideal candidate has an unrestricted CT RN license, a nursing degree, and at least 2 years of home health or hospice RN experience with strong documentation skills.
