Job SummaryWe are seeking a
Remote House Calls Nurse Practitioner in NJ, OH, PA, NE, TX to support our in-home medical services program. This role provides
virtual clinical oversight, patient assessments, care coordination, and provider support
for patients receiving care in their homes.
This is a fully remote position with a strong focus on quality, efficiency, and patient-centered care.
Responsibilities- Conduct
telehealth visits
for patients enrolled in the House Calls program
- Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patientrequiresprimary care services
- Perform
care management needs assessments, follow-ups, and care plan adjustments
- Review patient histories, labs, imaging, and diagnostic results
- Support
medication management and reconciliation
- Coordinate care with primary care providers (if patients already have one), specialists, and care management teams
- Document visits and clinical decisions accurately in the EMR
- Ensure compliance with clinical protocols, quality standards, and regulatory requirements
- Participate in interdisciplinary case reviews and quality improvement initiatives
- Active
Nurse Practitioner (NP) license
in the required state(s)
- National NP certification (ANCC or AANP)
- Minimum2+ years of clinical experience
as a Nurse Practitioner
- Experience in
house calls, home health, geriatrics, primary care, or internal medicine
strongly preferred
- Prior
telehealth or remote care experience
preferred
- Strong clinical judgment and comfort managing complex patients remotely
- Excellent communication and documentation skills
- Must be licensed in NY and one of the other states listed or willing to obtain
Technical & Regulatory Requirements
- Proficiency with
EMR/EHR systems
- Comfortable using telehealth platforms and remote monitoring tools
- Active
DEA registration
(or willingness to obtain, if applicable)
