The Long Term Care Nurse Practitioner will provide comprehensive clinical care to patients in long-term care and post-acute facilities in Kansas City Kansas. This full-time, role includes on-site visits to skilled nursing facilities, assisted living facilities, and similar settings, as well as some work-from-home responsibilities such as documentation, telehealth follow-ups, and care coordination. Day-to-day tasks include performing assessments, managing chronic and acute medical conditions, prescribing and administering medications, ordering and interpreting diagnostic tests, and developing individualized care plans in collaboration with facility staff and families.

The Nurse Practitioner will provide preventive care and patient/family education, participate in interdisciplinary team meetings, and ensure accurate, timely documentation in the electronic health record. The role also involves working closely with physicians and other advanced practice providers to support continuity of care and uphold Wellspring Healthcare’s standards for compassionate, patient-centered service.

• Demonstrated expertise in Nursing and Medicine, with strong clinical judgment appropriate for geriatric and post-acute care settings.
• Hands-on experience in Long-term Care, including management of complex chronic conditions and familiarity with skilled nursing and assisted living environments, desired but not required.
• Strong Critical Thinking skills to assess changing patient conditions, prioritize care, and make sound, evidence-based decisions.
• Proficiency in Medication Administration, including prescribing within scope of practice, monitoring for side effects, and ensuring safe medication management.
• Current Nurse Practitioner license and national certification (e.g., FNP, AGNP) in good standing, with eligibility to practice in Kansas and Missouri or willingness to obtain licensure.
• Experience or strong interest in geriatrics, post-acute, or long-term care populations.
• Ability to work effectively in a hybrid model, including facility-based care and remote documentation, with reliable transportation for on-site visits.
• Excellent communication and collaboration skills to work with patients, families, facility staff, and an interdisciplinary care team.
• Comfort with electronic health records and basic digital tools for documentation
