Wellstar Health System is seeking an experienced Hospice Admission RN to travel to patient sites within Wellstar campus areas to enroll patients in hospice care. The role requires educating patients and families, performing assessments, and coordinating the initial plan of care.
2 years RN with hospice care preferred but will consider motivated RNs committed to hospice. Strong empathy and patient-centered communication are essential for this in-home/home-care oriented role.
