About This Role

This role is ideal for a compassionate Registered Nurse looking to make a significant impact in the community by conducting assessments and helping members access necessary care and services. The position involves flexible work from home with substantial travel requirements.

• Location: Trenton, NJ
• Schedule: Full-time
• Sign-On Bonus: New Hire Bonus Available!
• Residency: Must reside in Morris County, New Jersey
• Travel: Up to 85% travel required

• Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
• Help members access vital services such as Adult Medical Daycare and Personal Care Assistance.
• Complete 10+ assessments per week using a holistic approach.
• Collaborate with your team and supervisor to overcome barriers and achieve goals.
• Use interviewing skills to engage members and uncover their health needs.
• Mentor new team members once you’ve mastered your role.
• Assist with developing a proactive plan of care to enhance member outcomes.
• Assess the need for referrals to clinical resources and interdisciplinary team members.

Education:

• Associate Degree in Nursing required; BSN preferred.

Experience:

• 1 or more years clinical experience (hospital, home health, or ambulatory care).

Licenses &

Certifications:

• Active, unrestricted RN license, New Jersey required.
• Successful completion of the NJ Choice Certification required.
