Midtown Manhattan, NY (Field-Based)

Schedule:

Full-Time, 35 Hours/Week, Monday-Friday Compensation: $110,000-$140,000+ annually, based on experience and credentials Benefits:

This is an outstanding opportunity for an experienced Hospice Registered Nurse to join a respected and growing healthcare organization dedicated to delivering exceptional end-of-life care. Key Responsibilities
  • Manage a caseload of hospice patients within the assigned territory.
  • Conduct comprehensive nursing assessments and ongoing clinical evaluations.
  • Monitor patient conditions, symptoms, and response to treatment plans.
  • Develop, implement, and revise individualized plans of care.
  • Provide education, guidance, and emotional support to patients, families, and caregivers.
  • Coordinate medications, medical equipment, supplies, and ancillary services.
  • Collaborate closely with physicians, social workers, chaplains, and the interdisciplinary hospice team.
  • Maintain accurate, timely, and compliant EMR documentation.
  • Participate in interdisciplinary team meetings, admissions, recertifications, and care planning activities.
  • Provide backup on-call coverage approximately 2-3 times per month.
  • Participate in one weekend rotation per month.
Qualifications Required
  • Current New York State Registered Nurse (RN) license in good standing.
  • Strong clinical assessment, critical thinking, and care coordination skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work independently in a field-based environment.
