Schedule:
Full-Time, 35 Hours/Week, Monday-Friday Compensation: $110,000-$140,000+ annually, based on experience and credentials Benefits:

This is an outstanding opportunity for an experienced Hospice Registered Nurse to join a respected and growing healthcare organization dedicated to delivering exceptional end-of-life care. Key Responsibilities- Manage a caseload of hospice patients within the assigned territory.
- Conduct comprehensive nursing assessments and ongoing clinical evaluations.
- Monitor patient conditions, symptoms, and response to treatment plans.
- Develop, implement, and revise individualized plans of care.
- Provide education, guidance, and emotional support to patients, families, and caregivers.
- Coordinate medications, medical equipment, supplies, and ancillary services.
- Collaborate closely with physicians, social workers, chaplains, and the interdisciplinary hospice team.
- Maintain accurate, timely, and compliant EMR documentation.
- Participate in interdisciplinary team meetings, admissions, recertifications, and care planning activities.
- Provide backup on-call coverage approximately 2-3 times per month.
- Participate in one weekend rotation per month.
- Current New York State Registered Nurse (RN) license in good standing.
- Strong clinical assessment, critical thinking, and care coordination skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently in a field-based environment.
