The Registered Nurse – Home Health – Weekend Option – Full Time Day Shift role is a full-time, on-site position based in Springfield, MO, providing home health nursing care to patients in the community. Day-to-day responsibilities include conducting comprehensive nursing assessments, developing and implementing individualized care plans, administering medications and treatments, and monitoring patient responses. The nurse educates patients and caregivers on disease management, medications, and safety in the home, while coordinating with physicians, therapists, and other team members to ensure continuity of care.
This role also involves documenting clinical encounters accurately, participating in case conferences, and responding to changes in patient condition to support positive health outcomes during weekend day shifts.
- Active Registered Nurse (RN) license in the state of Missouri and graduation from an accredited nursing program.
- Experience in home health, medical-surgical, or related acute or post-acute care settings.
- Strong clinical assessment, critical thinking, and care planning skills aligned with home-based patient needs.
- Effective communication and teaching abilities to support patients, families, and interdisciplinary team members.
- Ability to work independently in patient homes, manage time efficiently, and adapt to varying care environments.
- Basic computer proficiency for electronic health record documentation and using mobile health technology.
- Valid driver’s license, reliable transportation, and ability to travel to patient homes in and around Springfield, MO.
- Weekend availability for day shift schedules; flexibility to meet patient and team needs.
- BLS/CPR certification and familiarity with infection control and patient safety standards.
