Home Health Registered Nurse (RN) – 36‑Hour Weekly Guarantee | No Weekends/On‑Call

This role is for a dedicated Registered Nurse (RN) Case Manager providing home‑based clinical care across Nash, Edgecombe, and Wilson Counties
. The position offers a 36‑hour weekly guarantee
, no weekend/on‑call requirements
, and mileage reimbursement per contract company
. Pay Rate: $50-$55/hr

• RN Case Management — Oversee a full patient caseload, complete assessments, coordinate care plans, and ensure continuity of care.
• Skilled Nursing Visits — Perform wound care, medication administration, disease‑management teaching, and safety evaluations.
• Care Coordination — Communicate with physicians, therapists, and other care team members to support patient outcomes.
• Documentation & Compliance — Maintain timely, accurate charting within agency and regulatory guidelines.
• Patient & Family Education — Teach medication adherence, symptom monitoring, and home safety strategies.

• Weekly Hour Guarantee: 36 hours
• Shift: Days — 8:00 AM to 4:30 PM
• Coverage Area:
  Nash, Edgecombe, Wilson Counties
• Mileage:
  Reimbursed between visits (per contract company)
• On‑Call/Weekends:
  None

• Active RN License — Verify RN licensure
