Job Summary:Carease Health - IL is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team of dedicated healthcare professionals. As a hybrid role, you will have the flexibility to work on-site with our skilled nursing facility patients while also working remotely from home supported by a dedicated scribe team for efficient documentation.
Responsibilities:- Provide high-quality preventative care to residents in skilled nursing facilities
- Build strong relationships with facility staff and maintain a patient-centered approach
- Collaborate with our clinical team to develop and implement individualized care plans
- Conduct on-site visits and remote work to ensure seamless patient care
- Utilize remote patient monitoring (RPM) and chronic care management (CCM) to minimize hospitalizations and improve patient outcomes
- Active NP or PA license in the state of practice
- 1+ year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
- Ability to thrive in a fast-paced environment with flexibility to adapt to changing priorities
- Competitive annual salary range: $27,000 - $29,000
