Nurse Practitioner (NP) - Part-Time

Carease Health - IL is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to join our team of dedicated healthcare professionals. As a hybrid role, you will have the flexibility to work on-site with our skilled nursing facility patients while also working remotely from home supported by a dedicated scribe team for efficient documentation.

• Provide high-quality preventative care to residents in skilled nursing facilities
• Build strong relationships with facility staff and maintain a patient-centered approach
• Collaborate with our clinical team to develop and implement individualized care plans
• Conduct on-site visits and remote work to ensure seamless patient care
• Utilize remote patient monitoring (RPM) and chronic care management (CCM) to minimize hospitalizations and improve patient outcomes

• Active NP or PA license in the state of practice
• 1+ year of experience in Geriatric care and documentation review
• Excellent communication, collaboration, and teamwork skills
• Ability to thrive in a fast-paced environment with flexibility to adapt to changing priorities

• Competitive annual salary range: $27,000 - $29,000
