As a Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

We are seeking a part-time Nurse Practitioner (21 hours per week) to join our team. This role is an excellent opportunity to make a difference in patient care while enjoying a flexible schedule and work-life balance. As a valued member of our team, you will receive:

• Maintain accurate and timely documentation of patient visits and treatments

• Build strong relationships with facility staff and patients

• Participate in quality improvement initiatives to enhance patient care standards

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
