Travel Requirements- Primary coverage area includes Chevy Chase, MD and neighboring communities within approximately 20 miles
- Regular local travel required for patient visits, supervisory visits, and admissions
- Travel to other outlying service areas based on patient census, staffing needs, and business priorities is required for this role
- Mileage reimbursement provided in accordance with company policy

What You'll Do Champion Exceptional Patient Care- Conduct admissions, assessments, recertifications, and change-in-condition visits
- Develop and oversee individualized care plans
- Monitor patient progress and intervene proactively when clinical needs change
- Collaborate with physicians, caregivers, and interdisciplinary teams to achieve optimal outcomes
- Mentor, coach, and support field RNs and LPNs
- Conduct supervisory visits, chart audits, and competency evaluations
- Provide real-time clinical guidance and problem-solving support
- Foster a culture of accountability, excellence, and professional growth
- Ensure adherence to agency policies, regulatory standards, and clinical best practices
- Maintain accurate and timely documentation within the EMR platform
- Support quality initiatives, audits, and accreditation readiness
- Uphold the highest standards of patient privacy and HIPAA compliance
- Partner with hospitals, physicians, discharge planners, and referral sources
- Support smooth patient onboarding and continuity of care
- Serve as a trusted clinical ambassador in the community
Required Qualifications
- Active Maryland RN License (DC and/or VA a plus)
- 2+ years of nursing experience
- Experience supervising, mentoring, or leading clinical staff
- Strong assessment, care planning, and clinical decision‑making skills
- Valid driver's license and reliable transportation
- Excellent communication, organizational, and documentation abilities
- Home Health or Skilled Home Care experience
- OASIS experience
- Multi‑state licensure (MD, DC, VA)
- Experience with Alaya Care or other EMR systems
- Associate Degree in Nursing or higher
- Enjoys mentoring and inspiring others
- Takes initiative and thrives independently
- Communicates confidently with patients, families, and healthcare professionals
- Balances compassion with clinical excellence
- Finds purpose in helping patients remain safely at home
