Role Description

This full‑time, on‑site Family Nurse Practitioner role is based in Phoenix, AZ and focuses on providing primary care services to geriatric patients in residential communities and home settings. The practitioner will perform comprehensive health assessments, manage chronic and acute conditions, prescribe medications, and develop individualized care plans in collaboration with patients, families, and community staff. Daily responsibilities include monitoring patient progress, coordinating care with physicians and other healthcare professionals, documenting visits accurately, and ensuring continuity of care across independent, assisted, and memory care communities.

The role also involves patient education, preventive care, and timely response to urgent clinical needs within the practice’s scope. The practitioner will contribute to the ongoing development of best practices and participate in quality improvement initiatives to support safe, effective, and compassionate care.

Qualifications
  • Candidates should possess advanced clinical skills in Family Nurse Practitioner practice and Nursing, with experience managing geriatric populations preferred.
  • Candidates should possess strong competencies in Primary Care delivery, including health assessment, chronic disease management, and preventive care.
  • Candidates should possess knowledge and skills in Medicine relevant to adult and older adult care, including pharmacology, diagnostics, and evidence‑based treatment planning.
  • Candidates should possess the ability to provide appropriate Urgent Care within the primary care setting, including triage, stabilization, and referral when necessary.
  • Active and unencumbered Nurse Practitioner license in the state of Arizona, and national board certification as a Family Nurse Practitioner (or equivalent) required.
  • Excellent communication, interpersonal, and documentation skills, with the ability to work collaboratively across multidisciplinary teams and with residential community staff.
  • Demonstrated commitment to person‑centered, culturally sensitive care, and respect for the dignity and independence of geriatric patients.
  • Current CPR/BLS certification and familiarity with electronic health record (EHR) systems; experience in home‑based or long‑term care settings is beneficial.
