• Complete an initial, comprehensive assessment of the patient and family/caregiver living situation to determine care needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Utilize all elements of the nursing process and adhere to the state Nurse Practice Act.
• Establish and periodically revise individualized plans of care in collaboration with the certifying medical provider based on comprehensive assessment and patient goals.
• Administer medications and treatments as prescribed, and provide health‑care instructions per assessment and plan.
• Act as case manager when assigned, coordinating patient care for the assigned caseload.
• Document patient services and status timely and accurately within professional practice standards and reimbursement requirements.
• Communicate with medical providers regarding patient needs, obtain and receive orders, and report changes in condition.
• Teach patients and family/caregivers self‑care techniques and provide medication, diet, and other instructions as ordered.
• Collaborate with interdisciplinary team members, attending required staff and team meetings.
• Participate in on‑call duties as defined by policy and coordinate equipment and service arrangements.
• Supervise ancillary personnel; delegate responsibilities when required; and coordinate care with LPN/LVN under the clinical manager.
• Pursue personal growth and maintain professional knowledge through continuing education.
• Participate in quality improvement activities and other committees as assigned.
• Assume responsibility for personal growth and development with continued education to maintain competence.
• Meet productivity expectations and accommodate geographic coverage as needed.
• Attend orientation of new employees and preceptorship of students as assigned.

• Graduate of an accredited school of nursing.
• One to two years of recent acute‑care experience in an institutional setting and one to two years of recent home‑health experience preferred.
• Currently licensed as a Registered Nurse in the state(s) of planned practice.
• Possess and maintain CPR certification.
• Licensed driver with an insured automobile in good working order.
• Excellent observation, verbal, and written communication skills; problem solving, basic math skills; nursing competency.
• Self‑directed and capable of working with minimal supervision.
• Capable of prolonged walking or standing; able to lift, position, and transport patients; lift supplies; reach, stoop, bend, kneel, and crouch as required.
• Possess visual acuity and hearing sufficient to perform required nursing skills.

Coverage Area:
Burien/West Seattle‑King County.

Compensation: $45‑$55 per hour.

Shift: Day.

Work Schedule:

Monday – Friday, 8:00AM to 5:00PM.
