Remote LPN Clinical Navigator Chronic Care

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to deliver virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will conduct outreach, coaching, and care coordination to close gaps in care, documenting interactions in Cadence and collaborating with the clinical team to keep patients informed and engaged.

The role is full-time and remote, based in the United States, with standard hours 8:00 AM to 5:00 PM or 9:00 AM to 6:00 PM in the Pacific
