  The pay range reflects the current compensation range for this position at the time of posting. Individual offers are determined based on directly related experience, skills, education, internal equity, and other job-related factors. This position plays a critical role in supporting Augusta Health’s mission and advancing departmental goals through measurable performance indicators and service excellence. This position contributes to a collaborative, patient-centered environment and helps drive outcomes aligned with organizational priorities.

The Hospice Admission Coordinator, Social Worker will work with the hospital, home health, other facilities (hospitals, ALF, SNF) and hospice team to coordinate care across the continuum. The hours are mostly daylight with occasional evenings and weekends as required to meet patient and family needs. Hospital consults and facilitation of patients coming onto hospice will be the primary focus of the role, however, there may be a need for home-based (patient home) visits.
