Deliver high-quality care directly to patients homes as a Nurse Practitioner conducting risk assessments throughout Hamilton County, TN. In this dynamic per diem position, you will enjoy remarkable flexibility, setting your own hours and working as much or as little as you prefer within the week.
Schedule:
- Flexible shifts available Monday through Sunday
- Enjoy the autonomy to create your own schedule
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform home-based risk assessments, primarily for Medicare Advantage plan members
- Assess patients medical history, conditions, and current medication regimens
- Deliver thorough education to patients regarding their medical status and medications
- Recognize and communicate any gaps in care, offering clear recommendations
- Complete documentation following each approximately 45-minute assessment
- No requirements for creating treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Tennessee
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Active CPR certification (BLS)
- At least six months of recent Nurse Practitioner experience is preferred
For this role, you will enjoy competitive compensation earn $80 to $90 per evaluation, plus a mileage stipend and additional pay for administrative duties associated with your visits.
