Geriatric APP role available with growing primary care team! Monday-Friday, no weekends/evenings, managing chronic conditions of a high-risk patient panel in a value-based team setting.

The Advance Practice Providers (NP/PA) we seek are those who have directly related experience in home health, geriatrics, and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
Qualifications- Licensed as a NP or PA in the State where Harmony Cares Medical Group office located
- Federal DEA License/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver’s license and good driving record
