Registered Nurse

Job Summary:

The Clinical Care Registered Nurse (CCRN) provides direct nursing care to participants, managing complex clinical situations independently using advanced nursing knowledge and skills. The role involves triaging emergent situations, monitoring acute and chronic medical conditions in coordination with Primary Care Providers, and delivering clinic and home-based care including treatments, procedures, and medication administration. Regular home visits for wound care, lab draws, and assessments are required.

The position demands strong collaboration with licensed and non-licensed personnel and adherence to core values of compassion, integrity, teamwork, and excellence.

• Triaging emergent clinical situations independently.
• Monitoring acute and chronic medical conditions in coordination with Primary Care Providers.
• Utilizing care plans to provide clinic and home-based direct nursing care.
• Administering treatments, procedures, and medications as prescribed.
• Conducting regular home visits to provide wound care, lab draws, nursing assessments, and other nursing tasks.
• Communicating effectively with staff, participants, caregivers, and external organizations.
• Maintaining visible presence in the care center and managing frequent interruptions.
• Working beyond normal hours, including weekends and holidays, as necessary.

• Current Registered Nurse license with Nursing Diploma or Degree from an accredited school or university.
• Minimum of one year of experience working with frail, elderly, or long-term care populations.
• Basic Life Support (CPR) certification by the American Heart Association or willingness to obtain.
• Reliable transportation with a valid Michigan driver's license and car insurance.
• Ability to work independently and manage complex clinical situations.
• Must be fully vaccinated or willing to be vaccinated against Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis

• Bachelor's degree in Science of Nursing preferred;
  Associate's degree required.
• Minimum one year of home care experience preferred.
• Minimum one year of wound care experience preferred.

• Work environment includes frequent interruptions and visible presence in the care center.
• Must demonstrate core values of compassion, integrity, teamwork, and excellence.
• Must be at least 18 years old.
• Work beyond normal hours including weekends and holidays may be required.

• Monday through Friday, 8:00 AM to 4:30 PM shift.
• Occasional extended hours, weekends, and holiday shifts as needed.
