Monday–Friday, 8:00 AM–4:30 PM Position Summary The Nurse Practitioner supports the Medical Director in guiding hospice and palliative care services at Ever Heart Hospice. This role provides direct patient care, clinical consultation, symptom management, and leadership while collaborating with physicians, care teams, community providers, staff, and volunteers to ensure compassionate, high-quality care across home, inpatient, hospital, and nursing facility settings.

Responsibilities of Position:

Palliative Care:

Collaborate with the attending physician to help palliative care patients to manage and plan for disease progression.

Collaborate with referring physician concerning assessment findings, diagnostic and therapeutic measures.

Evaluate quality of life and responses to treatments.

Perform Palliative Care consults to assess the patient, provide symptom management, educate regarding the disease process, coordinate providers, end of life planning, and link patient to available resources to enhance quality of life.

Develop agency policies, procedures, and protocols for Palliative Care.

Additional Responsibilities Support and update patient plans of care in collaboration with physicians, the Medical Director, and the interdisciplinary team.

Provide clinical input, oversight, and medication management within scope of licensure and in alignment with hospice and palliative care philosophy.

Collaborate with Admissions, Team Managers, and care teams regarding referrals, admissions, patient needs, and care coordination.

Serve as a clinical resource and liaison to physicians, hospitals, community providers, and specialty partners.

Support physician outreach, education, in-services, and presentations to increase understanding of hospice and palliative care services.

Assist with developing and reviewing clinical protocols, patient care policies, and procedures.

Maintain knowledge of palliative care needs for patients with terminal and serious illnesses.

Complete documentation in compliance with agency policy, accreditation standards, and regulatory requirements.

Perform other duties as assigned.

Qualifications Master’s Degree in Nursing, MSN, from an accredited School of Nursing.

Completion of an accredited Nurse Practitioner program and current Nurse Practitioner license in Ohio.

Current hospital privileges within the agency’s service area, or ability to obtain as required.

Minimum of five years of nursing experience, preferably in palliative care, hospice, oncology, geriatrics, intensive care, or family practice.

Valid driver’s license and ability to travel to patient homes and care sites.

Preferred experience includes nonprofit healthcare, home-based or facility-based care, hospice Conditions of Participation, and Ohio healthcare regulations.

Willingness to travel to patient homes, hospitals, nursing facilities, and agency sites, including during inclement weather.

Ability to lift up to 25 pounds and perform physical activities such as sitting, standing, walking, bending, twisting, turning, and lifting.
