Health Map Solutions is seeking a Registered Nurse, Care Navigator/Case Manager to lead case management for kidney health, coordinating care and improving outcomes for a mixed caseload. You will document in Compass, perform health screenings, reconcile medications, and maintain patient-centered care plans.
Ideal candidates will have an active Oregon RN license and 6+ years of RN experience including 3+ years in case management. CCM and dialysis/transplant experience are pluses.
