RN -Hospice Per Diem Nurse

A compassionate jobseeker is needed for the Hospice Nurse opportunity with our company! As a Hospice Nurse, you will be responsible for overseeing the quality, expert care of your assigned hospice patients.
This opportunity offers $38.00 - $45.05 an hour based on experience!

What You’ll Do:

• Perform patient assessments for comfort and relief symptoms
• Establish and implement an individualized plan of care for each patient
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management
• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.

What You’ll Bring to Our Team:

• Minimum of one year nursing experience - medical/surgical, oncology, geriatric or related experience, hospice/home care experience preferred
• N. required; BSN Degree preferred
• CHPN or OCN preferred, BLS training
• NJ driver’s license and insurance coverage required
• Bilingual is a plus!
