** AMG is looking for a Nurse Practitioner to provide home-based primary and palliative care to elderly, frail, chronically ill, and homebound patients in independent living, assisted living, and residential home settings within an assigned geographic territory.

** Compensation and Schedule
*** Guaranteed $110,000 annual base salary
* Monthly production-based incentive opportunity
* Approximately 8–15 patient visits per day **(Salt Lake County)
*** Your base salary is guaranteed. Additional production compensation is earned by meeting established productivity and performance metrics as your patient panel develops and varies based on individual performance.

**** What You’ll Do
*** Perform comprehensive and follow-up medical evaluations and provide ongoing primary and palliative care.
* Evaluate and manage acute, chronic, and complex geriatric conditions, including frailty, dementia, cardiovascular disease, diabetes, and COPD.
* Manage symptoms, pain, medications, and quality-of-life concerns.
* Develop and regularly review individualized treatment and care plans with patients, families, and caregivers.
* Prescribe medications and treatments within scope of practice.
* Order and interpret laboratory testing, imaging, and other diagnostic studies as clinically appropriate.
* Educate patients and families about treatment options, advance care planning, hospice, and supportive services.
* Communicate and collaborate with patients, families, caregivers, facility staff, specialists, and interdisciplinary care team members.
* Coordinate specialty care, home health, therapy, hospice, ancillary services, and safe transitions between care settings.
* Maintain accurate and timely EMR documentation, including completion within 72 hours of each visit.
* Provide coverage for other practitioners within the care group as needed.
* Maintain compliance with organizational policies, professional standards, and regulatory requirements.
* Participate in continuing education and professional development.

Experience in geriatrics, primary care, internal medicine, accountable care organizations (ACO), assisted living, home-based care, dementia care, palliative care, or hospice is beneficial. Previous home-based care experience is not required.## Qualifications##

* ** Graduate degree from an accredited Nurse Practitioner program
* Current Nurse Practitioner board certification
* Current or eligible Utah APRN licensure
* Current DEA license
* Current CPR
* Prescriptive authority as required by Utah law
* Valid driver’s license and reliable transportation
* Strong clinical assessment and decision-making skills
* Excellent communication and documentation skills
