Structured Family Caregiving Assessment Nurse

Alice's Place, LLC is seeking a dependable, compassionate, and professional Assessment Nurse to join our Structured Family Caregiving (SFC) team.

• Complete initial and ongoing nursing assessments for Structured Family Caregiving participants
• Conduct in-home visits as assigned
• Assess participants' health status, functional needs, medications, and overall care needs
• Complete required nursing documentation accurately and on time
• Identify changes in condition and communicate concerns to the appropriate care team
• Provide caregiver education and support when needed
• Maintain participant confidentiality and follow HIPAA requirements
• Follow Alice's Place policies and applicable Medicaid/program requirements
• Travel to assigned participant homes throughout designated service areas

• Current Georgia nursing license
• RN or LPN with appropriate experience and qualifications for assigned assessments
• Experience working with the elderly and/or individuals with disabilities preferred
• Strong assessment and documentation skills
• Dependable transportation
• Valid driver's license and automobile insurance
• Excellent communication and organizational skills
• Must be dependable and able to complete documentation within required time frames
• Home health, Medicaid waiver, community-based services, or long-term care experience is a plus
