The MDS Coordinator (RN) is a key member of the interdisciplinary health care team responsible for coordinating the development, completion, accuracy, and transmission of resident assessments and person-centered care plans. This position ensures documentation and coding comply with federal and state regulations, support appropriate reimbursement, and reflect each resident’s needs, preferences, goals, and clinical status. The MDS Coordinator serves as a resource to residents, families, and team members throughout the assessment and care-planning process.

Otterbein Marblehead is situated on 47 acres of land two blocks from the shores of Lake Erie, and just outside the gates of Lakeside Chautauqua - in the heart of Ohio's northern shores and islands vacation destinations. Responsibilities:

• Coordinate and complete accurate, timely MDS assessments in accordance with federal and state requirements.
• Create, maintain, and update individualized, person-centered care plans, including rehabilitative programs, in collaboration with the interdisciplinary team.
• Review medical records and obtain information through resident, family, and staff interviews to support accurate MDS coding and care planning.
• Ensure the medical record contains documentation that supports assessment findings, coding, services, and reimbursement.
• Coordinate assessment data entry, submission, and transmission within required time frames.
