The Clinical Nurse Coordinator will work closely with 1-3 Providers in Outpatient Cardiology Practice Clinics. They are the mediator between the patient and physician. Providers rely on clinical nurse coordinator to convey important information to them about our patients and alert them to critical issues in a timely manner.

CAM, South County, West County.

Monday through Friday, generally about 8am-5pm. Our phones shut off at 4:30. No Weekends, No Nights, No Holidays.

Our Nurse Coordinators attends clinics with their Providers and if no clinic, they are able to work remotely, from home, once deemed ready, following completion of orientation/training. Potential for 1 day a week work from home.

Provides direct patient care, including assessment, planning, implementation and evaluation of patients; completes health history and physical examination on patients; conveys findings to physician. Coordinates patient care, orders diagnostic tests, conveys findings to physician, attends inpatient rounds, coordinates patient appointments and assists with referrals to outside sources. Assists physician and provides support with examinations, procedures and other processes related to direct patient care.

Performs a variety of nursing administrative duties, including compliance with environmental safety regulations, infection control standards and quality assurance. Acts as liaison and team leader by facilitating communication and coordination of care between patients, families and health team members; explains routines and procedures to patients and coordinates patient care information to appropriate sources.
