Remote LPN: Chronic Care Navigator Medicare

Cadence seeks a Clinical Navigator (LPN/LVN) to provide virtual chronic care support for Medicare patients enrolled in Cadence care programs. You will perform structured check-ins, coaching, and care coordination to close gaps in care, while documenting interactions in the Cadence platform.

This full-time remote position is based in the United States. You will collaborate with clinicians and patient support teams using Cadence Platform, Google Workspace, and Slack, leveraging AI-assisted tools
