Rise Me is seeking an experienced HSS Clinical Coordinator RN to join our remote care coordination team. This contract-to-hire role supports high-risk maternity Medicaid members and reports to the Medical Director, with state-level interaction.

The candidate will coordinate care, educate members, and document care plans, while collaborating with two RNs, a LPN, and a community health worker. Ideal candidates hold an active PA RN license, reside in SE PA, and have 3–5 years in maternity care
