Ohana Pacific Health is seeking a licensed practical nurse (LPN) to serve as a Care Coordinator. The role focuses on delivering clinical care coordination and telephonic Chronic Care Management under GP supervision.
Responsibilities include structured patient calls, care plan updates in the EHR, non-face-to-face time tracking, medication reviews, and escalation when needed. The position supports care teams in a mission-driven environment on Oahu.
