Monday through Friday, 8:00 AM to 5:00 PM EST Work Arrangement:
Remote administrative work 1 to 2 days per week with in-person field visits 3 to 4 days per week Field Coverage:
Candidates must be comfortable traveling up to a 2.5-hour one-way radius from their home residence for face-to-face member visits. Mileage and lodging are reimbursed.

The LTSS Service Care Manager will manage a caseload of healthcare members with Long-Term Support and Services (LTSS) needs. This member-facing position combines remote administrative work with extensive field work and requires regular face-to-face member visits, assessments, care coordination, service monitoring, and communication with members, caregivers, providers, physicians, discharge planners, and other healthcare professionals.

• Manage a caseload of healthcare members with LTSS needs
• Conduct monthly and quarterly member contact
• Complete member assessments with members, caregivers, or providers to obtain information regarding client status, support systems, and service needs for care plan development
• Document member assessments and notes
• Monitor delivery of services through in-person visits and telephonic contact
• Follow up with members, caregivers, and providers
• Authorize and coordinate referrals for services
• Ensure provider services are delivered without gaps
• Identify functional deficiencies in plans of care
• Assist in coordinating the development of informal or voluntary services for integration into the member care plan
• Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plans
• Coordinate acute care and long-term care services
• Assist members with filing and resolving complaints and appeals
• Conduct face-to-face member visits in the field
• Complete approximately 1 to 4 field visits per day depending on location and drive time
• Complete administrative responsibilities remotely
• Participate in virtual team meetings with cameras on
• Attend an in-person team meeting or training session approximately once per quarter

• RN and/or LCSW/LCSW-A/BSN/ADN licensure or qualification as applicable
• Valid driver's license
• 2+ years of physical health care management experience
• Willingness to perform extensive field work
• Willingness to travel for face-to-face member visits
• Ability to drive up to a 2.5-hour one-way radius from home residence when required
• Ability to work a Monday through Friday, 8:00 AM to 5:00 PM EST schedule
• Ability to work remotely and conduct administrative duties independently
• Ability to conduct in-person member visits
• Ability to complete member assessments
• Ability to coordinate healthcare and long-term care services
• Ability to communicate effectively with members, caregivers, providers, physicians, and other healthcare professionals

• 4 to 6+ years of physical health care management experience preferred
• Hospital case management experience preferred
• Home health experience preferred
• Discharge planning experience preferred
• Long-term care experience preferred
• Licensed Clinical Social Worker experience preferred
• Registered Nurse experience preferred

• Training schedule is Monday through Friday, 8:00 AM to 5:00 PM EST
• Training and learning journey is approximately 3.5 weeks
