Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our Home Infusion team in San Bernardino, CA. This is a fantastic opportunity for a motivated nurse to make a meaningful impact by delivering high-quality infusion therapy directly to patients in the comfort of their own homes. Responsibilities:- Administer IV infusion therapies including antibiotics, TPN, hydration, and specialty medications in the home setting
- Perform comprehensive patient assessments and monitor responses to infusion treatments
- Educate patients and caregivers on infusion therapy, medication management, and safety protocols
- Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)
- Accurately document patient care, clinical findings, and treatment plans in compliance with regulatory standards
- Collaborate with physicians, pharmacists, and the care team to coordinate and adjust patient treatment plans
- Respond to patient concerns and on-call needs in a timely and professional manner
- Active and unrestricted California RN license required
- Minimum 1-2 years of IV/infusion therapy or home health nursing experience preferred
- Proficiency in peripheral IV insertion and central line access (PICC, port, tunneled catheters)
- Strong clinical assessment and critical thinking skills
- Valid California driver's license, reliable transportation, and current auto insurance
- CPR/BLS certification required
- Excellent communication skills and ability to work independently in a home care environment
- Compassionate, patient-centered approach with strong organizational skills
