Healthcare United is seeking a Registered Nurse/Case Manager for a comprehensive home-based health care company committed to providing at-home services in the comfort of the clientâÂÂs home.
Locations in Stratford, Hamden, Norwalk, and Middletown.
Full-Time/Part-Time
- Flexible Hours
- Choose 4/40 or 5 days! Per-Diem
- Must Case Manage 10 patients and complete at least 5 assessments per month.
- Graduate of an accredited Registered Nurse program
- Current license as a Registered Nurse in Connecticut
- Ability to assess and respond to needs of patients and families in varied settings.
- Ability to make meaningful observations and write comprehensive accurate reports.
- Generous sign-on bonus for full-time 40 hours
