Community Health Nurse ; Local Government

This position supports the health, safety, and care of older adults through consultation, assessment, and education in both office and community settings. As a Community Health Nurse 2, you will perform the following duties:

• Nursing Consultation:
  Provide guidance to staff on client cases across various programs
• Assessment Support:
  Assist care managers by providing insight into medications, diseases, conditions, and cognitive health
• Health Evaluation:
  Observe and assess individuals and families to ensure appropriate services are offered
• Eligibility Determination:
  Complete Level of Care assessments, Functional Eligibility Determinations, and PASRR II evaluations
• Home Visits:
  Conduct home assessments for medically unstable clients referred by care management staff
• Safety Review:
  Identify safety and health hazards in homes and document related efforts

• Full-time employment
• Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
• Telework:
  You may have the opportunity to work from home (telework) part-time after six months, depending on operational needs. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Eddystone. The ability to telework is subject to change at any time.

QUALIFICATIONS

• Two years of community health nursing experience and a bachelor's degree in nursing, including or supplemented by the required number of credit hours of community health nursing education, accredited by the National League for Nursing (a master's degree in nursing or public health accredited by the National League for Nursing or the American Public Health Association may be substituted for one year of community nursing experience;
  
  or
• Any equivalent combination of experience and training.

• Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or possession of a non-renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.

• Employees possessing nonrenewable temporary practice permits must obtain licensure as a Registered Nurse within the one (1) year period as defined by the Pennsylvania State Board of Nursing.

• You must be able to perform essential job functions.

• This position falls under the provisions of the Older Adult Protective Services Act.
• Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
