We are seeking an experienced Remote Oncology/Hematology Registered Nurse to provide telephonic triage, patient support, care coordination, and education throughout the cancer care continuum. The RN will assess patient needs, identify high-risk conditions, coordinate with multidisciplinary care teams, provide symptom-management guidance, and ensure timely access to appropriate care and resources. This position requires strong oncology knowledge, clinical judgment, communication skills, and the ability to work independently in a remote environment.
Key Responsibilities- Serve as a consistent point of contact for patients and caregivers throughout the cancer care continuum.
- Conduct telephonic assessments and determine the appropriate level of care.
- Provide education regarding symptoms, medication side effects, treatment plans, and self-management.
- Coordinate care with physicians, nurses, and other members of the multidisciplinary healthcare team.
- Identify clinical, financial, logistical, and psychosocial barriers to care and connect patients with appropriate resources.
- Facilitate outpatient and/or telehealth visits when symptoms can be appropriately managed in those settings.
- Recognize changes in patient condition and appropriately escalat[e] concerns.
- Accurately document assessments, interventions, communications, and care coordination activities in the EHR.
- Utilize multiple technology, communication, and reporting platforms to support efficient remote patient care.
- Maintain compliance with applicable nursing standards, organizational policies, and state scope-of-practice requirements.
- Participate in quality initiatives and ongoing professional development.
- Provide compassionate, patient-centered support while maintaining confidentiality and professionalism.
- Active and unrestricted Registered Nurse (RN) license in Texas or another compact/multistate state.
- Minimum 3 years of clinical nursing experience.
- At least 2 recent years of dedicated oncology experience, specifically in a Heme/Onc clinic, outpatient oncology practice, or hospital oncology unit.
- Strong oncology assessment, symptom-management, and clinical triage skills.
- Excellent verbal and written communication skills.
- Strong critical-thinking and problem-solving abilities.
- Experience with electronic health records and telecommunication technology.
- Ability to work independently and effectively manage time in a remote environment.
- Strong organization and ability to manage multiple priorities in a fast-paced setting.
- Ability to work Monday–Friday, 7:00 AM–4:00 PM Central Time.
- Must reside within the Greater Houston area and be able to travel to The Woodlands, TX for required onsite training and meetings.
- Reliable internet connection and a private, HIPAA-compliant workspace at home.
- OCN certification or willingness to obtain certification within one year.
- Bachelor of Science in Nursing (BSN).
- Previous experience with telephonic triage.
- Previous remote nursing or telehealth experience.
- Experience coordinating care for oncology/hematology patients.
- Oncology Certified Nurse (OCN) certification.

This position requires candidates to have recent dedicated oncology experience and an active compact/multistate RN license. Candidates must also be located within the Greater Houston area and able to attend required onsite training and meetings in The Woodlands, Texas.
