UPMC Specialty Care Infectious Disease is seeking a Licensed Practical Nurse (LPN), OP to join our team in Harrisburg, PA. This position offers a unique opportunity to combine traditional outpatient clinical responsibilities with OPAT (Outpatient Parenteral Antibiotic Therapy), care coordination, and laboratory monitoring.
The LPN will serve as a key liaison between providers, patients, infusion centers, home care agencies, home health nurses, and laboratory services. The ideal candidate will have strong clinical judgment, exceptional attention to detail, and the ability to effectively coordinate care for patients receiving long-term antibiotic therapy.

Schedule:
- Monday through Friday, 8:00 a.m. to 4:30 p.m.
- No weekends
- No holidays
- Approximately 90% onsite with some flexibility for remote work when responsibilities are limited to laboratory monitoring and care coordination
Ideal Candidate:
The ideal candidate will have experience with care coordination, laboratory monitoring, and patients receiving antibiotic or infusion therapy. Experience with OPAT (Outpatient Parenteral Antibiotic Therapy), PICC line management, care management, and coordinating services with infusion centers and home health agencies is highly preferred. Strong communication, organization, critical-thinking skills, attention to detail, and follow-through are essential, along with the ability to collaborate effectively with providers and multidisciplinary healthcare teams.
Purpose: Provides professional nursing care to office patients under supervision of patient care providers.
Responsibilities:
- Draw blood.
- Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health).
- Administer medications including oral, SQ, IM and IV drip.
- Assist physicians with examinations, diagnostic procedures and treatments.
- Assess patient's general condition and take vital signs and weight.
- Comply with all policies and procedures.
- Perform IV therapy under direction of Registered Nurse.
- Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record.
- Maintain strict confidentiality related to medical records and other data.
- Promote wellness by providing patient education materials, communicating physician advice/instructions.
- Graduate of an approved LPN school of nursing.
- One year clinical experience preferred.
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Licensed Practical Nurse (LPN)
- Act 34
Licensure, Certifications, and Clearances: CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire
* Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
