Health Snap is seeking a licensed practical nurse (LPN/LVN) to join our Care Navigator team supporting patients in chronic care management and remote patient monitoring. This full-time role runs Monday through Friday, 9:00 am–5:30 pm ET, with remote work options within a collaborative care model.
You will educate patients about their conditions, help set provider-approved goals, document care plans, and communicate with the care team to improve adherence and outcomes.
