Pay Rate: $40.26/hour - Weekly pay, W-2
Travel:
Approximately 80% travel
Mileage Reimbursement: $0.725 per mile

Monday through Friday, 8:00 AM to 5:00 PM EST
Work Arrangement:
Remote administrative work 1 to 2 days per week with in-person field visits 3 to 4 days per week
Field Coverage:
Candidates must be comfortable traveling up to a 2.5-hour one-way radius from their home residence for face-to-face member visits. Mileage and lodging are reimbursed.
Position Overview:
The LTSS Service Care Manager will manage a caseload of healthcare members with Long-Term Support and Services (LTSS) needs. This member-facing position combines remote administrative work with extensive field work and requires regular face-to-face member visits, assessments, care coordination, service monitoring, and communication with members, caregivers, providers, physicians, discharge planners, and other healthcare professionals.
- Manage a caseload of healthcare members with LTSS needs
- Conduct monthly and quarterly member contact
- Complete member assessments with members, caregivers, or providers to obtain information regarding client status, support systems, and service needs for care plan development
- Document member assessments and notes
- Monitor delivery of services through in-person visits and telephonic contact
- Follow up with members, caregivers, and providers
- Authorize and coordinate referrals for services
- Ensure provider services are delivered without gaps
- Identify functional deficiencies in plans of care
- Assist in coordinating the development of informal or voluntary services for integration into the member care plan
- Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plans
- Coordinate acute care and long-term care services
- Assist members with filing and resolving complaints and appeals
- Conduct face-to-face member visits in the field
- Complete approximately 1 to 4 field visits per day depending on location and drive time
- Complete administrative responsibilities remotely
- Participate in virtual team meetings with cameras on
- Attend an in-person team meeting or training session approximately once per quarter
- RN and/or LCSW/LCSW-A/BSN/ADN licensure or qualification as applicable
- Valid driver's license
- 2+ years of physical health care management experience
- Willingness to perform extensive field work
- Willingness to travel for face-to-face member visits
- Ability to drive up to a 2.5-hour one-way radius from home residence when required
- Ability to work a Monday through Friday, 8:00 AM to 5:00 PM EST schedule
- Ability to work remotely and conduct administrative duties independently
- Ability to conduct in-person member visits
- Ability to complete member assessments
- Ability to coordinate healthcare and long-term care services
- Ability to communicate effectively with members, caregivers, providers, physicians, and other healthcare professionals
- 4 to 6+ years of physical health care management experience preferred
- Hospital case management experience preferred
- Home health experience preferred
- Discharge planning experience preferred
- Long-term care experience preferred
- Licensed Clinical Social Worker experience preferred
- Registered Nurse experience preferred
Training schedule is Monday through Friday, 8:00 AM to 5:00 PM EST. Training and learning journey is approximately 3.5 weeks. Camera must remain on during training. Candidates must be available for the entire training period. No time off is permitted during the 3.5-week training period. Candidates must understand that they cannot miss training.
Field Work and Travel RequirementsThis is a member-facing field position. Approximately 80% of the role involves travel. In-person visits are generally scheduled 3 to 4 days per week. Remote administrative work is generally scheduled 1 to 2 days per week. Field workload may range from 1 to 4 visits per day. Longer drive times may result in only 1 member visit being completed that day.
