Geriatric Nurse Practitioner - Telehealth; Longevity Program) Telehealth Family Medicine

This clinician will provide telehealth longevity and geriatric care for adults 65+, with a focus on comprehensive assessment, cognitive screening, chronic disease management, and medication review. Visits include initial assessments and ongoing follow-ups, with additional responsibilities in advance care planning and GLP-1 management for appropriate patients. The role includes close clinical support from a board-certified geriatrician and work within a Medicare-billed care model.

• Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk.
• Provide medical follow-up visits for chronic disease management and ongoing care.
• Perform cognitive screening and staging, and elevate for further assessment when indicated.
• Prescribe and manage GLP-1 therapy for appropriate patients.
• Conduct advance care planning conversations.
• Document care in the EHR to support accurate Medicare billing.

• Active, unrestricted NP license in New York.
• Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging.
• Active DEA registration.
• Experience with Medicare documentation and E/M coding.
• Comfort with telehealth and EHR documentation.
• Reliable high-speed internet and a private, HIPAA-compliant workspace.

• Experience with comprehensive geriatric assessment.
• Familiarity with cognitive screening instruments such as Mini-Cog or MoCA.
• GLP-1 prescribing experience.
