Remote Telehealth NP Chronic Care & Hypertension

Belle is seeking a compassionate Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries in our CCM program. You will assess members, offer evidence-based recommendations, and coordinate with nurses, PCPs, and care coordinators to keep members healthy and out of the ED when appropriate.
