Registered Nurse – Case Manager

RN Case Manager, responsible for patient-centered care coordination to improve health outcomes.

• Assess, plan, implement, monitor, and evaluate care options to optimize patient health.
• Develop individualized care plans, integrating clinical guidelines and promoting quality and efficiency.
• Collaborate with patients, families, and healthcare teams for coordinated services.
• Monitor progress through diagnostics, utilization patterns, and metrics; adjust plans as needed.
• Manage utilization and practice metrics to enhance clinical, quality, and fiscal outcomes.
• Maintain thorough documentation of case management activities.
• Assist in designing and evaluating advanced patient-centered care models.

• At least 2 years of RN work experience; case management experience preferred.
• Certified Case Manager/ACMA certification preferred.
• BSN preferred; knowledge of healthcare laws, regulations, insurance policies, and healthcare IT systems is helpful.
• Ability to work independently and collaboratively within a multidisciplinary team.
