Health Snap is seeking New York-licensed LPNs to support patients in chronic care management and remote patient monitoring. This full-time, remote role operates Monday–Friday, 9:00am–5:30pm ET, collaborating with care teams to educate patients and empower them to manage their chronic conditions.
You will work with Health Snap’s remote patient monitoring platform, maintain patient records, and communicate with providers following approved workflows.
